To enroll, please download and / or complete the enrolment form and, if applicable, email/fax Medical Device Division the completed enrolment form (email: mdd@dh.gov.hk, fax no.: 3157 1286). Seats are limited and will be allocated on a first-come-first-served basis.


(The deadline of application for enrolment is 31 May 2024. The Medical Device Division will notify successful applicants of their application results by 07 Jun 2024. Those who do not receive the confirmation may assume that their applications are unsuccessful.)
